Alfacalcidol Oral Drops
Description:
Alfacalcidol Oral Drops 2 mcg/ml is a prescription vitamin d analogue medicine used to manage disorders of calcium and phosphate metabolism, especially in patients with kidney and parathyroid disorders. Each 10 ml bottle contains a concentrated solution of alfacalcidol, an active metabolite of vitamin d that is rapidly converted in the body to calcitriol, the hormonally active form of vitamin d3. It is commonly used in adults and children under medical supervision to treat hypocalcaemia, hypoparathyroidism, renal osteodystrophy, osteomalacia and related bone and mineral metabolism conditions. These oral drops allow precise dose adjustment in mcg for patients who require individualized vitamin d analogue therapy.
Indications:
- Treatment and prevention of hypocalcaemia (low blood calcium levels) associated with chronic kidney disease and other causes
- Management of hypoparathyroidism (reduced parathyroid hormone secretion) and pseudohypoparathyroidism
- Treatment of renal osteodystrophy (bone disease secondary to chronic renal failure)
- Treatment of osteomalacia (softening of the bones) due to vitamin d deficiency or impaired activation of vitamin d
- Management of hypophosphataemia (low blood phosphate levels) related to vitamin d metabolism disorders
- Supportive therapy in certain metabolic bone diseases where active vitamin d analogues are indicated, as prescribed by a physician
Active Ingredients / Key Components:
- Alfacalcidol – 2 mcg/ml oral solution (drops)
Dosage & Administration / How to Use:
- Use only as prescribed by a physician experienced in managing calcium and bone metabolism disorders.
- Adults (oral): Typical starting dose is 1 mcg once daily. Maintenance dose usually ranges from 0.25–1 mcg/day, adjusted according to serum calcium, phosphate and parathyroid hormone levels.
- Paediatric use: Dose must be individualized by the doctor based on age, weight, clinical condition and laboratory results.
- Measure the dose carefully using the dropper or dosing device provided (if supplied) or a calibrated oral syringe, according to your doctor’s instructions.
- Can be taken with or without food, preferably at the same time each day for consistent blood levels.
- Do not change the dose or stop the medicine without consulting your doctor, as abrupt changes may affect calcium and phosphate balance.
- Regular monitoring of serum calcium, phosphate, alkaline phosphatase and sometimes parathyroid hormone is essential; your doctor may adjust the dose or temporarily stop treatment if hypercalcaemia occurs.
Clinical Benefits:
- Helps normalize calcium and phosphate levels in patients with impaired vitamin d activation, especially in chronic kidney disease.
- Supports bone mineralization and helps reduce symptoms of osteomalacia such as bone pain, muscle weakness and increased fracture risk.
- Improves management of renal osteodystrophy by providing an active vitamin d analogue that bypasses renal activation.
- Assists in the control of hypoparathyroidism by improving calcium absorption from the gut and reducing bone demineralization.
Warnings & Contraindications:
- Contraindicated in patients with hypercalcaemia, hyperphosphataemia (except when associated with hypoparathyroidism), vitamin d toxicity, or known hypersensitivity to alfacalcidol or any component of the formulation.
- Use with extreme caution in patients with a history of nephrolithiasis (kidney stones), severe atherosclerosis, sarcoidosis or other granulomatous diseases, and in those on digitalis (cardiac glycosides).
- Frequent monitoring of serum calcium, phosphate and renal function is mandatory during therapy, especially at the start of treatment and after any dose adjustment.
- Concomitant use with other vitamin d products, calcium supplements, or thiazide diuretics may increase the risk of hypercalcaemia; your doctor may need to adjust doses.
- Consult your doctor before use if pregnant or breastfeeding; use only if the potential benefit justifies the potential risk to the mother and baby.
- Patients with severe liver impairment should be monitored carefully, as conversion to active metabolites occurs partly in the liver.
- Consult your doctor before use if you have kidney disease, heart disease, arrhythmias, or are taking medications such as digitalis, diuretics, anticonvulsants or corticosteroids.
- Not intended to diagnose, treat, cure, or prevent any disease without medical supervision.
- Keep out of reach of children.
Side Effects:
- The most important potential side effect is hypercalcaemia (high blood calcium), which may present with nausea, vomiting, constipation, loss of appetite, excessive thirst, frequent urination, headache, drowsiness, muscle weakness or bone pain.
- Other possible side effects include hyperphosphataemia, pruritus (itching), rash, abdominal pain, and, rarely, kidney stones or deterioration of kidney function.
- Prolonged hypercalcaemia may lead to vascular and soft tissue calcification; immediate medical attention is required if symptoms of high calcium occur.
- Stop the medication and seek urgent medical advice if you experience severe weakness, confusion, irregular heartbeat, severe abdominal pain, or persistent vomiting.
