It contains the fixed-dose combination of sacubitril and valsartan, known as an angiotensin receptor neprilysin inhibitor (ARNI), and is indicated to reduce the risk of cardiovascular death and heart failure hospitalizations in adult patients with chronic heart failure (NYHA class II–IV). Entresto is not a vitamin or dietary supplement; it is a potent prescription-only medicine that must be used under the supervision of a cardiologist or qualified healthcare professional.

  • Usual adult maintenance dose: 97 mg sacubitril / 103 mg valsartan (one 200 mg tablet) taken orally twice daily, as prescribed by a cardiologist.
  • Initiation: Recommended starting dose in most patients is 49 mg/51 mg twice daily, then increased after 2–4 weeks to 97 mg/103 mg twice daily if tolerated. Do not start directly at 200 mg unless specifically directed by your physician.
  • With or without food: Tablets may be taken with or without food, but should be taken consistently the same way each day.
  • Swallow whole: Swallow the tablet whole with a glass of water; do not crush or chew unless advised otherwise by your doctor.
  • ACE inhibitor washout: If switching from an ACE inhibitor, a washout period of at least 36 hours is required before starting Entresto to reduce risk of angioedema.

Warnings & Contraindications:

  • Do not use during pregnancy; medicines acting on the renin-angiotensin system can cause injury and death to the developing fetus. If pregnancy is detected, discontinue Entresto as soon as possible.
  • Breastfeeding: Use is generally not recommended; consult your doctor to weigh risks and benefits.
  • May interact with ACE inhibitors, ARBs, aliskiren, diuretics, NSAIDs, lithium, and other antihypertensives. Inform your doctor about all medicines and supplements you are taking.
  • Use with caution in patients with a history of angioedema, severe aortic or mitral stenosis, or obstructive hypertrophic cardiomyopathy.
  • Consult your doctor before use if pregnant, nursing, taking medications, or have medical conditions such as kidney disease, liver disease, low blood pressure, or high potassium levels.
  • Keep out of reach of children.

Side Effects:
  • Common side effects: Low blood pressure (dizziness, lightheadedness, fainting), increased blood potassium (hyperkalemia), kidney function changes, cough, fatigue, headache, and gastrointestinal discomfort.
  • Serious side effects: Angioedema (swelling of face, lips, tongue, or throat; difficulty breathing or swallowing), severe hypotension, severe kidney impairment, or dangerously high potassium levels. These require immediate medical attention.
  • If you experience swelling of the face, lips, tongue, or throat, sudden difficulty breathing, severe dizziness, or fainting, stop taking the medicine and seek emergency medical help immediately.
  • Always report any unusual or persistent symptoms to your doctor, especially if you have underlying kidney disease or are on multiple heart medications.
