About the product

Mounjaro 5 mg Pen

Description:

Mounjaro 5 mg Pen is a prescription injectable medicine containing tirzepatide, a dual GIP (glucose-dependent insulinotropic polypeptide) and GLP-1 (glucagon-like peptide-1) receptor agonist. It is used for the management of type 2 diabetes mellitus and for medical weight management in adults, as an adjunct to diet and exercise. This pre-filled subcutaneous pen delivers a fixed 5 mg dose of tirzepatide in 0.5 ml solution and is designed for once-weekly administration under medical supervision. Mounjaro helps improve blood glucose control and supports clinically significant weight loss in appropriate patients.

Indications:

  • Treatment of adults with insufficiently controlled type 2 diabetes mellitus as an adjunct to diet and exercise:
    • As monotherapy when metformin is considered inappropriate due to intolerance or contraindications.
    • In combination with other medicinal products for the treatment of diabetes (e.g. insulin, other oral antidiabetics) as directed by a physician.
  • Weight management in adults as an adjunct to a reduced-calorie diet and increased physical activity, including weight loss and weight maintenance, in patients with:
    • Body Mass Index (BMI) ≥ 30 kg/m2 (obesity), or
    • BMI ≥ 27 kg/m2 to < 30 kg/m2 (overweight) in the presence of at least one weight-related comorbid condition such as hypertension, dyslipidaemia, obstructive sleep apnoea, cardiovascular disease, prediabetes, or type 2 diabetes mellitus.
  • Improvement of glycaemic control (fasting and postprandial blood glucose) in adults with type 2 diabetes.

Active Ingredients / Key Components:

  • Tirzepatide – 5 mg per pre-filled pen (5 mg/0.5 ml solution for injection, concentration 10 mg/ml).
  • Other excipients: Full excipient list is not provided in the available information (For full ingredient information, please check the product leaflet or the manufacturer’s official website.). Please refer to the official package leaflet for a complete list of inactive ingredients.

Dosage & Administration / How to Use:

  • General dosing:
    • Starting dose: 2.5 mg tirzepatide once weekly for 4 weeks (using the appropriate starting strength pen, not this 5 mg pen).
    • After 4 weeks, the dose is increased to 5 mg once weekly. Mounjaro 5 mg Pen is typically used as the first maintenance step after the initial 2.5 mg titration.
    • If additional glycaemic or weight control is needed, the dose may be increased in 2.5 mg increments (e.g. 7.5 mg, 10 mg, 12.5 mg, 15 mg) after a minimum of 4 weeks on the current dose, as directed by the treating physician.
    • Recommended maintenance doses: 5 mg, 10 mg, or 15 mg once weekly.
    • Maximum dose: 15 mg once weekly.
  • Route of administration:
    • Subcutaneous injection only. Do not administer intravenously or intramuscularly.
    • Inject into the abdomen, thigh, or upper arm.
    • Rotate injection sites with each dose to reduce the risk of lipodystrophy or local irritation.
  • Timing of dose:
    • Administer once weekly, on the same day each week, at any time of the day.
    • Can be given with or without food.
  • Use with insulin:
    • If used together with insulin, inject Mounjaro and insulin at separate injection sites (different areas of the body or at least a few centimeters apart).
    • Blood glucose monitoring and possible adjustment of insulin dose may be required to reduce the risk of hypoglycaemia.
  • Missed dose:
    • If a dose is missed and the next scheduled dose is more than 3 days (72 hours) away, administer the missed dose as soon as possible.
    • If the next scheduled dose is within 3 days, skip the missed dose and take the next dose on the regular day. Do not take a double dose.
  • Administration instructions (general guidance):
    • Always check the label to ensure you are using the correct strength (5 mg pen).
    • Inspect the solution visually: it should be clear and free from particles. Do not use if cloudy, discolored, or contains particles.
    • Clean the injection site with an alcohol swab and allow to dry.
    • Follow the pen’s specific instructions for removing the cap, priming (if required), and injecting the full dose as per the manufacturer’s patient leaflet.
    • Dispose of used pens in an approved sharps container according to local regulations.
  • Use only under the supervision and prescription of a qualified healthcare professional.

Clinical Benefits:

  • Improves glycaemic control in adults with type 2 diabetes by enhancing glucose-dependent insulin secretion and reducing elevated blood glucose levels.
  • Promotes clinically meaningful weight loss and supports long-term weight management when combined with a reduced-calorie diet and increased physical activity.
  • Reduces appetite and caloric intake through dual GIP and GLP-1 receptor agonism, supporting better adherence to dietary goals.
  • May contribute to reductions in cardiometabolic risk factors associated with obesity and type 2 diabetes (such as high blood sugar, excess body weight, and related complications), as demonstrated in clinical studies.

Warnings & Contraindications:

  • Mounjaro (tirzepatide) is a potent prescription medicine and should only be used under medical supervision.
  • Contraindicated in patients with known hypersensitivity to tirzepatide or any of the excipients.
  • Not indicated for use in patients with type 1 diabetes mellitus or for the treatment of diabetic ketoacidosis.
  • Use with caution in patients with a history of pancreatitis; discontinue if pancreatitis is suspected (e.g. persistent severe abdominal pain, sometimes radiating to the back).
  • Gastrointestinal adverse reactions (nausea, vomiting, diarrhoea) may occur, particularly during dose escalation; dehydration may result from severe vomiting or diarrhoea.
  • May increase the risk of hypoglycaemia when used in combination with insulin or insulin secretagogues (e.g. sulfonylureas); dose adjustment of concomitant therapy may be required.
  • Limited data are available in patients with severe renal or hepatic impairment; careful monitoring and dose adjustment may be necessary as advised by a physician.
  • Thyroid-related warnings: GLP-1 receptor agonists as a class have been associated with thyroid C-cell tumours in rodents; the relevance to humans is unknown. Use with caution in patients with a personal or family history of medullary thyroid carcinoma or MEN 2, as advised by a specialist.
  • Pregnancy and breastfeeding: Use is not recommended during pregnancy or lactation unless clearly needed and prescribed by a physician. Discuss potential risks and benefits with your doctor.
  • Not recommended in children and adolescents; safety and efficacy in patients under 18 years of age have not been established.
  • Consult your doctor before use if pregnant, nursing, taking other medications, or if you have medical conditions such as pancreatitis, severe gastrointestinal disease, kidney disease, liver disease, thyroid disease, or cardiovascular disease.
  • Keep out of reach of children.
  • Not intended to diagnose, treat, cure, or prevent any disease without a valid medical prescription and supervision.

Side Effects:
  • Common side effects: Nausea, vomiting, diarrhoea, constipation, decreased appetite, indigestion, abdominal pain, and injection site reactions (redness, itching, or swelling).
  • Less common side effects: Headache, dizziness, fatigue, belching, gastroesophageal reflux, and mild dehydration (especially if significant vomiting or diarrhoea occurs).
  • Serious side effects (seek immediate medical attention): Signs of pancreatitis (persistent severe abdominal pain, with or without vomiting), severe allergic reactions (rash, itching, swelling of face/lips/tongue, difficulty breathing), severe dehydration (dizziness, fainting, reduced urination), or symptoms suggestive of thyroid problems (neck swelling, difficulty swallowing, hoarseness, shortness of breath).
  • This is not a complete list of side effects. Always read the patient information leaflet and consult your doctor or pharmacist if you experience any unusual or persistent symptoms.
