About the product›
Orap Forte 4 Mg 20 Tab
Description:
Orap Forte 4 mg 20 Tab is a prescription antipsychotic medicine containing pimozide, a diphenylbutylpiperidine derivative. It is primarily used for the long-term management of chronic psychotic disorders and for the control of severe motor and vocal tics in Gilles de la Tourette syndrome. Pimozide works mainly by selectively blocking dopamine receptors in the brain, helping to reduce abnormal thoughts, behaviors, and involuntary movements. This product is intended for use under strict medical supervision and is not a self-medication treatment.
Indications:
- Chronic psychotic disorders, as prescribed by a psychiatrist or neurologist.
- Gilles de la Tourette syndrome (Tourette’s disorder), particularly for the control of severe motor and vocal tics when other treatments are inadequate or not tolerated.
- Management of symptoms such as hallucinations, delusions, disorganized thinking, and severe behavioral disturbances associated with chronic psychoses (as directed by a specialist).
Active Ingredients / Key Components:
- Pimozide – 4 mg per tablet
- Other inactive ingredients (excipients) are not specified in the available information (For full ingredient information, please check the product leaflet or the manufacturer’s official website.).
Dosage & Administration / How to Use:
- Use only as prescribed by a physician experienced in the treatment of psychotic disorders or Tourette syndrome.
- Typical dosing is individualized based on the patient’s age, condition, response to treatment, and concomitant medications. The total daily dose and titration schedule must be determined by the doctor.
- Tablets are usually taken orally with a glass of water, with or without food, at the same time each day, or as directed by the physician.
- Do not exceed the prescribed dose and do not stop treatment abruptly without consulting your doctor, as sudden discontinuation may worsen symptoms.
- If a dose is missed, take it as soon as you remember unless it is almost time for the next dose. Do not double the dose to make up for a missed one.
- Regular medical follow-up, including ECG monitoring and evaluation of electrolytes, may be required because pimozide can prolong the qt interval.
Clinical Benefits:
- Helps control persistent psychotic symptoms such as hallucinations, delusions, and severe behavioral disturbances in chronic psychoses.
- Reduces frequency and intensity of motor and vocal tics in patients with Gilles de la Tourette syndrome.
- Improves overall daily functioning, social interaction, and quality of life in appropriately selected patients under specialist care.
- Provides a long-acting antipsychotic effect through selective dopamine receptor blockade and inhibition of dopamine reuptake.
Warnings & Contraindications:
- Pimozide can prolong the qt interval and may cause serious cardiac arrhythmias. Concomitant use with other qt-prolonging drugs (such as certain antiarrhythmics, phenothiazines, tricyclic antidepressants) should be avoided.
- Use with caution in patients with a history of cardiovascular disease, arrhythmias, electrolyte disturbances (hypokalemia, hypomagnesemia), or those taking medicines that affect heart rhythm.
- May lower the convulsive threshold; caution is advised in patients with epilepsy or those taking anticonvulsants.
- Concomitant use with alcohol or other central nervous system depressants (hypnotics, sedatives, strong analgesics, anxiolytics, etc.) may potentiate sedation and impair judgment and motor skills.
- Not recommended in patients with known hypersensitivity to pimozide or any component of the formulation.
- Should be used with caution in patients with liver impairment, kidney impairment, Parkinson’s disease, or a history of blood dyscrasias, as advised by the physician.
- Consult your doctor before use if pregnant or planning pregnancy. Use during pregnancy or breastfeeding should only occur if the potential benefit justifies the potential risk to the mother and baby.
- May cause drowsiness, dizziness, or impaired alertness. Patients should avoid driving or operating machinery until they know how the medicine affects them.
- Not intended for self-medication. Use only under medical supervision and regular monitoring.
- Keep out of reach of children.
Side Effects:
- Common side effects may include drowsiness, dizziness, dry mouth, constipation, blurred vision, weight changes, and fatigue.
- Extrapyramidal symptoms (movement disorders) such as tremor, rigidity, restlessness (akathisia), dystonia, or parkinsonian-like symptoms may occur, especially at higher doses.
- Endocrine-related effects such as increased prolactin levels may lead to menstrual disturbances, breast enlargement, or galactorrhea in some patients.
- Serious side effects may include qt prolongation, cardiac arrhythmias, severe dizziness or fainting, signs of neuroleptic malignant syndrome (high fever, muscle rigidity, confusion, autonomic instability), or severe allergic reactions.
- Seek immediate medical attention if you experience chest pain, palpitations, fainting, severe muscle stiffness, high fever, difficulty breathing, or swelling of the face, lips, or tongue.
