Description:

Saxenda 6 mg 1 pen (SAXENDA 18 mg/3 ml pre-filled pen s.c.) is a prescription injectable medicine used for chronic weight management. It contains liraglutide, a glucagon-like peptide-1 (GLP-1) receptor agonist, and is administered once daily by subcutaneous injection using a pre-filled, multi-dose pen. Saxenda is indicated as an adjunct to a reduced-calorie diet and increased physical activity in adults with overweight or obesity and specific weight-related comorbidities. It is a non-insulin therapy that can also help lower blood sugar and HbA1c in patients with type 2 diabetes who are using it for weight management under medical supervision.


Indications:

  • Chronic weight management in adult patients with an initial body mass index (bmi) of ≥30 kg/m2 (obese), or ≥27 kg/m2 (overweight) in the presence of at least one weight-related comorbid condition (such as hypertension, type 2 diabetes mellitus, or dyslipidemia), as an adjunct to a reduced-calorie diet and increased physical activity.
  • Helps reduce body weight and maintain weight loss in adults who respond to therapy.
  • As a GLP-1 receptor agonist, it may contribute to improved glycemic control and reduction in blood sugar and HbA1c in overweight or obese adults with type 2 diabetes when used under physician guidance for weight management.

Active Ingredients / Key Components:

  • Liraglutide – 18 mg per 3 ml pre-filled pen (concentration: 6 mg/ml).
  • Each pen is designed to deliver adjustable once-daily doses of 0.6 mg, 1.2 mg, 1.8 mg, 2.4 mg, or 3 mg of liraglutide via subcutaneous injection.

Dosage & Administration / How to Use:

  • Saxenda is for subcutaneous (s.c.) use only. Do not administer intravenously or intramuscularly.
  • Initiation dose: 0.6 mg subcutaneously once daily for 1 week.
  • Titration schedule: Increase the dose by 0.6 mg/day at weekly intervals as tolerated until a maintenance dose of 3 mg/day is achieved:
    Week 1: 0.6 mg once daily
    Week 2: 1.2 mg once daily
    Week 3: 1.8 mg once daily
    Week 4: 2.4 mg once daily
    Week 5 and onward (maintenance): 3 mg once daily (if tolerated).
  • If the patient cannot tolerate an increased dose during titration, consider delaying dose escalation for an additional week. If the 3 mg/day dose is not tolerated, discuss continuation or discontinuation with the prescribing physician.
  • Administer once daily at any time of day, with or without food, preferably at approximately the same time each day.
  • Injection sites: abdomen, thigh, or upper arm. Rotate injection sites within the same region to reduce the risk of lipodystrophy or skin irritation.
  • Use the pre-filled pen as instructed by your healthcare provider. Always attach a new sterile needle for each injection and dispose of used needles safely in a sharps container.
  • Do not share your Saxenda pen with anyone else, even if the needle is changed, to avoid cross-infection.
  • Follow your doctor’s instructions regarding duration of therapy. Treatment should be part of a comprehensive weight management program including diet, exercise, and lifestyle modification.

Warnings & Contraindications:

  • Boxed warning – Risk of thyroid C-cell tumors: Liraglutide has caused thyroid C-cell tumors, including medullary thyroid carcinoma (MTC), in rodent studies. It is unknown whether Saxenda causes thyroid C-cell tumors, including MTC, in humans. Saxenda is contraindicated in patients with a personal or family history of MTC or in patients with Multiple Endocrine Neoplasia syndrome type 2 (MEN 2).
  • Contraindicated in patients with a known hypersensitivity to liraglutide or any of the excipients.
  • Not recommended for use in patients with type 1 diabetes mellitus or for the treatment of diabetic ketoacidosis. It is not a substitute for insulin.
  • Use with caution in patients with a history of pancreatitis. Discontinue if pancreatitis is suspected (persistent severe abdominal pain, possibly radiating to the back, with or without vomiting).
  • May cause gallbladder disease (including cholelithiasis and cholecystitis); evaluate patients with abdominal pain suggestive of gallbladder issues.
  • Monitor renal function in patients with renal impairment, particularly if they experience severe gastrointestinal adverse reactions (nausea, vomiting, diarrhea), which may lead to dehydration and worsening renal function.
  • May increase heart rate; use cautiously in patients with pre-existing cardiovascular disease and monitor as directed by a physician.
  • Psychiatric effects: monitor for depression, suicidal thoughts, or unusual mood changes. Discontinue and seek medical advice if such symptoms occur.
  • Not recommended during pregnancy or breastfeeding. Weight loss offers no benefit to pregnant women and may harm the fetus. Discontinue Saxenda if pregnancy occurs.
  • Not indicated for use in pediatric patients; safety and efficacy in children and adolescents may differ and should only be considered under specialist guidance where approved.
  • Consult your doctor before use if you have thyroid disease, pancreatitis, gallbladder disease, severe gastrointestinal disease, kidney or liver problems, cardiovascular disease, or if you are taking other antidiabetic medications.
  • Do not use Saxenda together with other GLP-1 receptor agonists or with other liraglutide-containing products.
  • Keep out of reach of children.

Side Effects:
  • Common side effects: Nausea, vomiting, diarrhea, constipation, abdominal pain, decreased appetite, dyspepsia, headache, fatigue, dizziness, injection site reactions (redness, swelling, itching), and mild increase in heart rate.
  • Less common but important side effects: Gallstones, cholecystitis, pancreatitis, dehydration (due to severe vomiting or diarrhea), hypoglycemia (especially when used with other antidiabetic drugs such as sulfonylureas or insulin), and mild elevations in pancreatic enzymes.
  • Serious side effects – seek immediate medical attention: Signs of pancreatitis (severe, persistent abdominal pain with or without vomiting), signs of gallbladder disease (severe upper abdominal pain, fever, jaundice), symptoms of thyroid tumor (neck mass, difficulty swallowing, hoarseness, shortness of breath), severe allergic reactions (rash, itching, swelling of face/tongue/throat, severe dizziness, difficulty breathing), or severe dehydration (very dry mouth, excessive thirst, little or no urination, weakness).
  • If any side effects are severe, persistent, or concerning, stop using the product and consult your healthcare provider promptly.

Storage Instructions:

Store Saxenda pens in a refrigerator at 2–8 °c before first use. Do not freeze; discard if the pen has been frozen. Protect from direct sunlight and excessive heat. After first use, the pen may typically be stored at room temperature (up to the temperature specified in the official leaflet) or in a refrigerator, according to the manufacturer’s instructions. Keep the cap on when not in use. Do not use after the expiry date printed on the pack. Keep out of reach of children.
