Description:

Sinemet 25/250 mg is a prescription oral medicine used in the management of Parkinson's disease and Parkinsonism. Each tablet combines carbidopa and levodopa, two well-established anti-Parkinson agents that work together to increase dopamine levels in the brain and improve motor control. By inhibiting the peripheral breakdown of levodopa, Sinemet enhances the amount of levodopa that reaches the brain, helping to reduce tremor, rigidity, bradykinesia (slowness of movement), and other Parkinsonian symptoms. This product is intended for use under medical supervision only.


Indications:

  • Treatment of Parkinson's disease (idiopathic Parkinsonism).
  • Treatment of Parkinsonian syndromes, including post-encephalitic Parkinsonism and symptomatic Parkinsonism that may occur after carbon monoxide or manganese intoxication (as directed by a physician).
  • Relief of classic Parkinsonian symptoms such as resting tremor, muscular rigidity, bradykinesia, and postural instability.
  • Improvement of motor fluctuations and "on-off" phenomena in patients previously treated with levodopa alone, as assessed and adjusted by a neurologist or treating physician.

Dosage & Administration / How to Use:

  • Sinemet 25/250 mg must be used exactly as prescribed by your doctor; dosing is individualized based on disease severity, previous levodopa therapy, and patient response.
  • Typical adult dosing (for reference only – follow your doctor’s instructions): therapy is usually started with lower-strength combinations and titrated; Sinemet 25/250 mg is often used in patients already stabilized or requiring higher levodopa doses. Do not initiate, stop, or change dose without medical advice.
  • Tablets are taken orally with a glass of water. They may be taken with or without food; however, taking with a small, low-protein snack can help reduce nausea. High-protein meals may reduce the effect of the medicine and should be spaced from dosing as advised by your doctor.
  • Swallow tablets whole; do not crush or chew unless your doctor or pharmacist specifically instructs otherwise.
  • Take doses at the same times each day to maintain stable blood levels and symptom control.
  • If you miss a dose, take it as soon as you remember unless it is almost time for the next dose. Do not double the dose to make up for a missed one.
  • Do not stop Sinemet suddenly without medical supervision, as abrupt withdrawal may worsen symptoms or, rarely, cause serious complications such as neuroleptic malignant syndrome–like reactions.
  • Elderly patients or those with other medical conditions may require lower starting doses and slower titration as determined by the physician.

Warnings & Contraindications:

  • Use only under the supervision of a physician experienced in the management of Parkinson's disease.
  • Contraindicated in patients with known hypersensitivity to carbidopa, levodopa, or any component of the formulation.
  • Contraindicated in patients taking non-selective monoamine oxidase (MAO) inhibitors; at least 14 days should elapse between stopping a non-selective MAO inhibitor and starting Sinemet, unless otherwise directed by a doctor.
  • Not recommended in patients with narrow-angle glaucoma; use with caution in patients with wide-angle glaucoma, with regular intraocular pressure monitoring.
  • Use with caution in patients with severe cardiovascular or pulmonary disease, bronchial asthma, renal or hepatic impairment, endocrine disorders, or a history of peptic ulcer disease or convulsions.
  • May cause or worsen psychiatric symptoms such as depression, suicidal ideation, hallucinations, confusion, agitation, or psychosis, especially in elderly or predisposed patients. Close monitoring is required.
  • May cause sudden onset of sleep or excessive daytime sleepiness; patients should avoid driving or operating machinery until they know how the medicine affects them.
  • May cause orthostatic hypotension (drop in blood pressure on standing); stand up slowly and report dizziness or fainting to your doctor.
  • Use in pregnancy and breastfeeding only if clearly needed and after careful risk–benefit assessment by the treating physician. Consult your doctor before use if you are pregnant, planning pregnancy, or breastfeeding.
  • Use with caution in patients with a history of melanoma or suspicious undiagnosed skin lesions, as levodopa may activate malignant melanoma; regular skin examinations are recommended.
  • Consult your doctor before use if you have heart disease, arrhythmias, psychiatric illness, liver disease, kidney disease, or endocrine disorders.
  • Do not consume alcohol while taking Sinemet without medical advice, as it may increase certain side effects such as dizziness or drowsiness.
  • Keep out of reach of children.

Side Effects:
  • Common side effects may include nausea, vomiting, loss of appetite, dry mouth, dizziness, headache, and drowsiness.
  • Other possible effects include orthostatic hypotension (low blood pressure on standing), palpitations, irregular heartbeat, flushing, and sweating.
  • Neurological and psychiatric side effects may include dyskinesias (involuntary movements), confusion, hallucinations, insomnia, vivid dreams, agitation, anxiety, or mood changes.
  • Gastrointestinal effects may include abdominal pain, constipation, diarrhea, or dark discoloration of saliva, urine, or sweat (usually harmless but should be reported if concerning).
  • Rare but serious side effects can include severe allergic reactions (rash, itching, swelling, difficulty breathing), severe mental or behavioral changes, uncontrolled movements, chest pain, irregular heartbeat, or signs of gastrointestinal bleeding (black/tarry stools, vomiting blood).
  • Seek immediate medical attention if you experience severe dizziness, fainting, chest pain, difficulty breathing, severe confusion, high fever, muscle rigidity, or sudden worsening of Parkinsonian symptoms.
  • Report any unusual skin lesions or changes in existing moles to your doctor.
