Description:
Exforge Hct 10/160/25 mg tablets are a prescription antihypertensive medicine combining three active ingredients (amlodipine, valsartan, and hydrochlorothiazide) to effectively lower and control high blood pressure in adults. This triple combination targets blood pressure through complementary mechanisms: a calcium channel blocker, an angiotensin II receptor blocker (ARB), and a thiazide diuretic. Exforge Hct is used when blood pressure is not adequately controlled with dual therapy and helps reduce the risk of serious cardiovascular complications associated with uncontrolled hypertension, such as stroke and heart attack, when used as part of a comprehensive blood pressure management plan.
Indications:
- Treatment of essential hypertension (high blood pressure) in adult patients.
- Indicated in patients whose blood pressure is not adequately controlled by a combination of amlodipine and valsartan, valsartan and hydrochlorothiazide, or amlodipine and hydrochlorothiazide alone.
- Helps reduce elevated systolic and diastolic blood pressure, thereby lowering the risk of cardiovascular events such as stroke, myocardial infarction (heart attack), and related complications when used as directed by a physician.
- May be used as replacement therapy in patients already receiving the three components (amlodipine, valsartan, hydrochlorothiazide) from separate tablets at the same dose.

Dosage & Administration / How to Use:
- Exforge Hct 10/160/25 mg tablets are for oral use only.
- The usual recommended dose is one tablet once daily, or as prescribed by the treating physician based on the patient’s blood pressure response and clinical condition.
- Take the tablet at the same time each day, with or without food, and swallow whole with a glass of water. Do not crush or chew the tablet.
- Dosage must be individualized. Your doctor may adjust the dose or switch strengths depending on blood pressure control and tolerability.
- Do not stop taking Exforge Hct suddenly without consulting your doctor, as this may cause your blood pressure to rise again.
- If you miss a dose, take it as soon as you remember on the same day. If it is almost time for your next dose, skip the missed dose and continue with your regular schedule. Do not take a double dose to make up for a missed one.
- Use only under medical supervision, especially in patients with kidney impairment, liver impairment, or electrolyte disturbances.

Warnings & Contraindications:
- Contraindicated in patients with known hypersensitivity to amlodipine, valsartan, hydrochlorothiazide, other dihydropyridine calcium channel blockers, sulfonamide-derived drugs, or any component of the formulation.
- Contraindicated in pregnancy. Drugs acting on the renin–angiotensin system (such as ARBs) can cause injury and death to the developing fetus. If pregnancy is detected, discontinue Exforge Hct as soon as possible.
- Contraindicated in severe hepatic impairment, cholestasis or biliary obstruction, and in patients with anuria or severe renal impairment where diuretics are ineffective (hydrochlorothiazide component).
- Contraindicated in refractory hypokalemia, hypercalcemia, and symptomatic hyperuricemia or gout related to thiazide therapy.
- Use with caution in patients with renal artery stenosis, heart failure, recent myocardial infarction, aortic or mitral stenosis, hypertrophic cardiomyopathy, or severe hypotension.
- Hydrochlorothiazide may cause electrolyte disturbances (hypokalemia, hyponatremia, hypomagnesemia) and changes in blood glucose and uric acid; regular monitoring is recommended.
- May cause dizziness or fatigue, especially at the start of treatment or when the dose is increased. Avoid driving or operating machinery until you know how the medicine affects you.
- Consult your doctor before use if pregnant, nursing, planning pregnancy, taking other medications (especially other antihypertensives, potassium supplements, potassium-sparing diuretics, lithium, NSAIDs, or antidiabetic drugs), or if you have kidney disease, liver disease, diabetes, gout, electrolyte imbalances, or heart conditions.
- Not intended for use in children and adolescents unless specifically prescribed by a specialist.
- Keep out of reach of children.
Side Effects:
- Common side effects may include dizziness, headache, flushing, fatigue, ankle or leg swelling (peripheral edema), palpitations, nausea, abdominal discomfort, and mild hypotension.
- Hydrochlorothiazide-related effects may include increased urination, electrolyte imbalances (such as low potassium or sodium), increased blood uric acid, changes in blood glucose, muscle cramps, or weakness.
- Less common but serious side effects can include severe hypotension, angioedema (swelling of face, lips, tongue, or throat), severe allergic reactions, significant kidney function changes, arrhythmias, chest pain, or visual disturbances.
- Stop taking the medicine and seek immediate medical attention if you experience difficulty breathing, swelling of the face or throat, severe dizziness or fainting, chest pain, or signs of severe allergic reaction.
