Indications:

Dosage & Administration / How to Use:
- Exforge Hct 5/160/12.5 mg tablets is for oral use only and must be taken exactly as prescribed by your doctor.
- Usual adult dose: One tablet once daily, preferably at the same time each day. The exact dose and strength may be adjusted by the physician based on blood pressure response and clinical condition.
- May be taken with or without food. Swallow the tablet whole with a glass of water; do not crush or chew.
- Do not change your dose or stop taking the medication without consulting your doctor, even if you feel well, as hypertension often has no symptoms.
- If you miss a dose, take it as soon as you remember on the same day. If it is almost time for the next dose, skip the missed dose and continue with your regular schedule. Do not take a double dose to make up for a missed tablet.
- Regular monitoring of blood pressure, kidney function, and electrolytes (especially potassium and sodium) is recommended during treatment.

Warnings & Contraindications:

- Contraindicated in severe kidney impairment, anuria (inability to produce urine), or severe liver impairment, and in patients with refractory hypokalemia, hypercalcemia, or symptomatic hyperuricemia/gout related to thiazide diuretics.
- Use with caution in patients with kidney disease, liver disease, heart failure, aortic stenosis, hypertrophic cardiomyopathy, electrolyte disturbances (low sodium, low or high potassium, low magnesium), or a history of gout or high uric acid.
- May cause symptomatic hypotension (low blood pressure), especially in volume-depleted patients (e.g., those on high-dose diuretics, low-salt diet, diarrhea, or vomiting). Start treatment under medical supervision if you are at risk.
- Hydrochlorothiazide may increase blood sugar and cholesterol levels; use with caution in diabetic and dyslipidemic patients and monitor regularly.
- May increase sensitivity to sunlight (photosensitivity); avoid excessive sun exposure and use adequate sun protection.
- Drug interactions: Use with caution with other antihypertensives, potassium-sparing diuretics, potassium supplements, lithium, NSAIDs, and certain other medications. Inform your doctor and pharmacist about all medicines and supplements you are taking.
- Consult your doctor before use if pregnant, nursing, taking other medications, or if you have medical conditions such as kidney disease, liver disease, heart failure, diabetes, gout, or electrolyte imbalances.

Side Effects:

- Hydrochlorothiazide may cause increased urination, muscle cramps, low blood pressure, electrolyte disturbances, and in some cases increased blood sugar or uric acid.
- Valsartan and amlodipine may rarely cause significant hypotension, kidney function changes, or, in the case of amlodipine, gingival hyperplasia (gum overgrowth).
- Serious but less common side effects include severe allergic reactions, angioedema (swelling of face, lips, tongue, or throat), severe dizziness or fainting, chest pain, irregular heartbeat, severe abdominal pain, yellowing of the skin or eyes (jaundice), or signs of severe skin reactions.
- Stop taking the medicine and seek immediate medical attention if you experience difficulty breathing, swelling of the face or throat, severe dizziness, fainting, or any other severe or unusual symptoms.
